• NYC HEALTH+HOSPITAL / GOTHAM SCHOOL BASED HEALTH CENTER

    Dear Parent or Guardian:

    We are happy to inform you that your child’s school – Long Island City High School – has a School Based Health Center (SBHC)! The SBHC is staffed by Morrisania, a Gotham Health Center, licensed professionals consisting of medical and mental health providers.

    Please know that your child can use the School-Based Health Center and see your other doctors as well. Signing this consent does not change your insurance, does not change your private doctor, and does not affect the number of times your child can see their primary doctor.

    At the School Based Health Center, your child can receive the services listed below at no cost to you, regardless of insurance status. The SBHC is allowed to bill insurance, however there are no co-pays for you, and you do not receive a bill.

    School Based Health Center Services include:

    • Complete physical examinations
    • Medications and prescriptions
    • Medical laboratory tests; Immunizations
    • Medical care, including treatment for acute and chronic conditions
    • Age appropriate reproductive healthcare
    • Health Education and Counseling
    • Mental Health Counseling and services
    • Screening for vision, hearing, asthma, obesity, and other medical conditions;
    • Access to care 24 hours/day, 7 days/week

    To register your child for the services of our School Based Health Center, please read and complete the following information on the attached enrollment form. Be sure to sign the Parental Consent form.

    Give the completed forms to your principal’s office or directly to the School Based Health Center in room 546.

    The School Based Health Center is located in room 546 of your child’s school and is open every school day between the hours of 8:00am-4:00pm.

    Es un placer informarle que la escuela de su hijo/a–Long Island City High School – tieneun Centro de Salud Escolar. El Centro de Salud Escolar es administrado por Morrisania, a Gotham Health Center. El Centro de Salud Escolar tiene profesionales de Morrisaniacon licenciaque consiste de proveedores médicos y de salud mental.

    Tenga en cuenta que su hijo/apuede usar el centro de salud escolar y también ver a sus otros médicos. Su firma en esta autorización no cambia su plan de seguro médico, no cambia su doctor privado, y no afecta la cantidad de visitas a su médico privado.

    Su hijo/a puede recibir serviciosnotados aquíen nuestro Centro sin ningún costo a usted y sin importar si tiene o no seguro médico. Si su hijo/a tieneseguro médico, El Centro de Salud Escolar está permitido facturar a su compañía de seguro, sin embargo, no hay copagos para usted, y usted no recibiráuna cuenta.

    Para registrar su hijo/a para que recibanservicios médicos,un padre o un encargado debe leer y firmar los siguientes formularios.

    Por favor devuelvael formulario ala oficina del director de la escuela o directamente al Centro de Salud Escolar.

    El Centro de Salud Escolar se encuentra en la habitación 546 de la escuela de su hijo/a. Las horas son de lunes a viernes entre las 8:00am-4:00pm.
