• STUDENTS LEAVING EARLY

     

    We acknowledge that emergencies arise and students may need to leave early from school before the end of their official day.

     

    Students can only leave school early by being picked up by an adult who is listed on their Blue Emergency Contact Card (Blue Card)

     

    NOTE: Only adults named on the Blue Card may pick students up from school. This includes illness or medical emergency. Additional names can always be added to the back of the card. In the event of an emergency, students will ONLY be released to people whose names are listed on the Blue Card.  

     

    Adults coming into the building to pick up a student must show an ID to security in the main lobby, and sign in both at the security desk and the main office. Staff members will check to ensure that the adult is listed as one of the contacts before contacting the child and allowing them to be released.

     

    When the student comes back to school, they should be sent with a note from their guardian explaining the absence. The student must show the note to all teachers of classes that were missed, and finally turn it in with all relevant teacher signatures a thte end of their day after their last class to office 210.

    BLUE EMERGENCY CONTACT CARD (BLUE CARD)

     

    Updating your records with the school is extremely important. On your child’s first day of school orientation you will be asked to fill out a Blue Emergency Contact Card (Blue Card). Students will also be introduced to the Blue Card during their identity units the first week of school. You can also contact the parent coordinator for additional instructions on updating your records on file.

     

    NOTE: Only adults named on the Blue Card may pick students up from school. This includes illness or medical emergency. Additional names can always be added to the back of the card. In the event of an emergency, students will ONLY be released to people whose names are listed on the Blue Card.

     

    If you move, change your home or work phone number, please call the school to request and fill out a new Blue Emergency Contact Card. The information on these cards must be accurate at all times.

     

    Each person must be over 18 years of age and familiar to your child. If you wish for a babysitter or neighbor to be able to pick up your child in the event of an emergency, their name and phone number MUST appear on the emergency contact card.

     

    If you are a parent who lives with your child, or a custodial guardian, you can update your own home, cell, and work phone numbers and email address of yourself or other additional adults on your child’s record in your NYC Schools Account, using the Emergency Contact page, at schoolsaccount.nyc.

    You also have the option to update your preferred written or spoken language. You can also update your child’s health information to share with your child’s school. This includes your child’s physician name and phone number, any known diagnoses, allergies, limitations that may limit physical activity, and health insurance status.

     

    If you need to update your home address, update siblings on your child’s record, or update order of protection information, you MUST contact your school directly to make the change(s). We ask for your cooperation in completing these forms as soon as possible.
